Treatment consent and medical-history forms open with Jotform and Google Forms. Telemedicine, financing, pharmacy, nutrition and video links open other providers’ websites. Information you enter on those websites is handled by the relevant provider under its own privacy terms, together with any applicable clinic arrangements.
